Public Housing Application

Please read before completing the application:

Lake Metropolitan Housing Authority operates a Public Housing Program for the benefit of low income families, senior citizens (age 62 and older) and disabled persons. Lake Metropolitan Housing Authority manages three developments in Painesville, Ohio.

· Jackson Towers

· Washington Square

· Woodlawn Homes

Lake Metropolitan Housing Authority serves as the owner and landlord of the development and charges rent to the household based on 30% of the adjusted gross income.

Submitting Your Application:

Please complete the entire application. All adults who will be living in the assisted housing unit must sign, date & include their social security number on the application.

A complete application packet requires the following forms:

Please mail completed application to our office. Faxed copies will not be accepted

Complete the Background Investigation Authorization form

Complete the HUD form # 92006: Supplement to Application for Federally Assisted Housing form is on page 6.
Required Documentation forms

NOTE: All address changes and other updates to your application, must be submitted to LMHA in writing, within 10 days of the change. LMHA does not forward mail or accept any changes over the phone.
