Public Housing Applicant Forms

Please download and print the application.  Complete all questions and include social security numbers for anyone age 18 or over,  who will be  living with you in the assisted housing unit. Mail or deliver the completed application to Lake Metropolitan Housing Authority at 189 FIrst Street, Painesville, Ohio  44077.  Faxed applications will not be accepted. Also complete the Background Investigation Authorization form and the HUD form - Supplement to Application for Federally Assisted Housing and return these forms along with your application. After submitting your application, all address changes and updates must be in writing and sent to LMHA within 10 days of the change.  LMHA does not forward mail or take any changes over the phone.
