Work Orders: For all maintenance work requests (during and after business hours) please contact the LMHA office at (440) 354-3347 ext. 10. Any maintenance calls received after business hours must be an emergency. The list below describes several emergency situations.

Any condition that jeopardizes the security of the unit.

Absence of a working heating system when outside temperature is below 60 degrees Fahrenheit.

Utilities not in service, including no running hot water (not in service due to disconnection for non-payment is not an issue that can be resolved by LMHA maintenance staff).

Conditions that present the imminent possibility of injury.

Obstacles that prevent safe entrance or exit from the unit, such as lock out (with proper identification of resident), broken lock which effects unit security.

Broken window glass which affects unit security, is a cutting hazard, or occurs within inclement weather.

Plumbing leaks which have the capacity to create flooding or cause damage to the unit.

Natural gas leaks or smell of fumes (contact LMHA after you have contacted the Gas Company).

Any electrical problem or condition that could result in shock or fire.

Inoperable Smoke/CO2 detector(s).

Absence of a functioning toilet in the unit.

Ceiling damage caused by leak (only in the event that there is an immediate threat of collapse).

Public Housing Forms
Listed below are some of the Public Housing forms. Access these forms by clicking on the red title.
- Parking: Residents who own a vehicle must have the vehicle registered in their name and must obtain an LMHA parking sticker. All vistors must park in authorized vistor parking.
- Move Out Notice: Residents must provide intent to vacate no later than 30 days prior to their expected move out date. Please complete and sign the Move Out Notice form and return tho the LMHA office.
- Pet Policy: Pets or Service animals are to be leashed and under the owner's control at all times.
- Unlawful/Criminal Conduct: Residents and their guests, shall not engage in any unlawful or criminal conduct. Do not act in any way that will endanger anyone lawfully in the building. In the event of an emergency, call 911.
- Community Service: Each month, every non-exempt adult resident must contribute eight hours of community service or participate in an economic self-sufficiency program for eight hours each month. There are volunteer opportunities at many local organizations throughout Lake County. Please complete your monthly community service time sheet.
